Key Codes in the Demo Data

The demo carries the pack's four scenarios running side by side at Red Ridge Mine (MINE01).

Code What it is
CV-101 / CR-101 / EX-204 Discharge conveyor, jaw crusher, hydraulic excavator
ISO-2026-000318 SCN-01 — CV-101 GROUP isolation, 4 points, 6 workers on board GB-CPP-01. Status RESTORING (held by the lock event below)
ISO-2026-000321 SCN-02 — EX-204 PERSONAL isolation, attachment lowered, accumulator bled. Status ACTIVE
ISO-2026-000324 SCN-03 — CR-101 COMPLEX isolation held while testing. Status TESTING
TEST-2026-00042 Approved test release of the CR-101 HV point — persons clear, 30-second rotation, re-verification still outstanding
ISO-EVT-2026-00019 SCN-04HIGH: contractor left site with personal lock PL-104 still on the board. Restoration stopped → dual approval → forced removal
ISO-EVT-2026-00020 MODERATE: zero-energy verification FAILED at the CV-101 chute gate — retained material meant gravitational energy wasn't controlled
GB-CPP-01 Group isolation board, master lock GM-001, six participants
PL-101…PL-107 Personal locks (one per worker). PL-104 is the force-removed contractor lock
PTW-2026-001204/5 General work + lifting permits on CV-101 (simultaneous operations)
TMPL-CV-101 Approved group isolation template, Rev 3, 4 ordered steps
ISO-AUD-2026-0003 Half-yearly system audit, score 86.5, two open findings

Status Vocabularies (the important ones)

Isolation lifecycle: DRAFT → PLANNED → AWAITING_APPROVAL → APPROVED → PREPARING → APPLIED → VERIFIED → ACTIVE, then SUSPENDED ⇄ ACTIVE, TESTING ⇄ ACTIVE, ACTIVE → RESTORING → COMPLETED; plus CANCELLED and BREACHED

Permit lifecycle: DRAFT → REQUESTED → RISK_REVIEW → APPROVED → ISSUED → ACCEPTED → ACTIVE, then SUSPENDED ⇄ ACTIVE, EXTENDED ⇄ ACTIVE, ACTIVE → HAND_BACK → CLOSED; plus CANCELLED, EXPIRED, BREACHED

Isolation point status: IDENTIFIED · READY · ISOLATED · LOCKED · TAGGED · DISSIPATED · VERIFIED · TEST_RELEASED · RESTORED · EXCEPTION

Verification outcome: PASS · FAIL · NOT_POSSIBLE · NOT_REQUIRED — a FAIL holds the isolation and marks the step EXCEPTION

Lock status: AVAILABLE · ISSUED · APPLIED · REMOVED · LOST · DAMAGED · QUARANTINED Lock type: PERSONAL · GROUP_MASTER · EQUIPMENT · DEPARTMENT · CONTRACTOR · CONTROL · EMERGENCY · TEST

Event severity: LOW · MODERATE · HIGH · CRITICAL · POTENTIAL_FATALITY

Competency status: CURRENT · EXPIRING · EXPIRED · SUSPENDED · REVOKED

Rules of Thumb

  • Work does not start until the linked isolation is ACTIVE, every required step is VERIFIED, and no verification has failed.
  • An isolation cannot go ACTIVE with an unassessed energy source, an unrepresented isolation point, or undissipated stored energy — and re-accumulation risk must be addressed (can_reaccumulate).
  • A personal lock may only be removed by its owner. Anything else needs forced_removal plus a linked Approval — the demo shows the two-approver case (area owner + isolation officer).
  • Restoration is blocked while any permit is open, any worker is signed on, or any personal lock remains on the board.
  • A test release names its affected steps explicitly, requires persons-clear confirmation, and is not finished until the points are reapplied and re-verified.
  • Authorisations expire. An EXPIRING/EXPIRED competency should block safety-critical assignment (demo: a gas tester expiring 15 Aug 2026).
  • Approved evidence is immutable — correct by superseding record, not by edit.
