Rules of Thumb
- Work does not start until the linked isolation is
ACTIVE, every required step isVERIFIED, and no verification has failed. - An isolation cannot go
ACTIVEwith an unassessed energy source, an unrepresented isolation point, or undissipated stored energy — and re-accumulation risk must be addressed (can_reaccumulate). - A personal lock may only be removed by its owner. Anything else needs
forced_removalplus a linkedApproval— the demo shows the two-approver case (area owner + isolation officer). - Restoration is blocked while any permit is open, any worker is signed on, or any personal lock remains on the board.
- A test release names its affected steps explicitly, requires persons-clear confirmation, and is not finished until the points are reapplied and re-verified.
- Authorisations expire. An
EXPIRING/EXPIREDcompetency should block safety-critical assignment (demo: a gas tester expiring 15 Aug 2026). - Approved evidence is immutable — correct by superseding record, not by edit.
